Before you complete the application, we have a few requirements to consider and prepare before you submit.

As a vendor, we require you to submit your certificate of insurance naming the entities below as certificate holders.

If you do not meet the insurance requirements listed below, you must email Kristin Fowler at *lifestyle@gcprai.com* after submitting your application for further evaluation; please be prepared to submit a copy of your personal automotive insurance policy and/or any applicable certifications (i.e. Texas Cottage Law- Food Handlers Certification etc.)

Failure to complete one of these steps may result in denial of your vendor application.

Insurance Requirements: Commercial General Liability (CGL)
$1,000,000 Occurrence
$1,000,000 Personal Injury
$1,000,000 Products/Operation
Auto Liability (owned/non owned) - $500,000/$1,000,000 Bodily Injury
Workers Compensation - Statutory Limits
